We accept applications on a rolling basis throughout the year.

Applications will be reviewed on the following schedule:

• Complete applications received by:

  • June 26th 2026 will be placed or waitlisted by July 30th 2026

  • September 4th 2026 will be placed or waitlisted by September 18th 2026

  • October 30th 2026 will be placed or waitlisted by December 4th 2026

  • January 29th 2027 will be placed or waitlisted by February 26th 2027

  • March 5th 2027 will be placed by March 26th 2027

How Do I Apply?

 ALL APPLICANTS COMPLETE STEPS 1 & 2

STEP 1

REGISTRATION FORM

This form must be filled out even if you have attended sessions at our clinic before.

STEP 2

$340 DEPOSIT AND $50 NONREFUNDABLE PROCESSING FEE

• After completing the registration form, check your email for a message from Social Thinking Stevens Creek inviting you to sign into your Client Portal

  • Please allow 2-3 business days for this email to arrive

• You can submit payment for the deposit and processing fee through the Client Portal or you can call 408-244-2005 to pay by credit card

  Your card will not be charged until your application is complete

STEP 5

RECENT PICTURE OF YOUR CHILD

MOST RECENT REPORTS

• Please send any of the following (if applicable):

  • Individualized Education Program (IEP)

  • 504 Plan

  • Psycho-educational assessment

  • Neuropsychological evaluation

  • Speech and language assessment/report

  • Doctor’s letter OR report including medical diagnosis (not required but helpful if seeking insurance reimbursement)
