How Do I Apply?
ALL APPLICANTS PLEASE COMPLETE STEPS 1 & 2

REGISTRATION FORM
This form must be filled out even if you have attended sessions at our clinic before.
STEP 2
$320 DEPOSIT AND $50 NONREFUNDABLE PROCESSING FEE
After completing the registration form, check your email for a message from Social Thinking Stevens Creek inviting you to sign into your Client Portal
Please allow 2-3 business days for this email to arrive
You can submit payment for the deposit and processing fee through the Client Portal or you can call 408-244-2005 to pay by credit card

TEACHER QUESTIONNAIRE
Please send this link https://forms.gle/WUKhjiJ4gUkFa3x1A to as many of your child’s teachers as possible. This form is available as a PDF upon request.
STEP 5
RECENT PICTURE OF YOUR CHILD
A SHORT VIDEO OF YOUR CHILD (2-3 MINUTES)
This might include your child:
Pretend playing with someone else (e.g., parent, sibling, friend, etc.)
Having a conversation with someone about a recent event or experience (e.g., day at school, trip to the beach, etc.)
MOST RECENT REPORTS
Please send any of the following (if applicable):
Individualized Education Program (IEP)
504 Plan
Psycho-educational assessment
Neuropsychological evaluation
Speech and language assessment/report
Doctor’s letter OR report including medical diagnosis (not required but helpful if seeking insurance reimbursement)

Important Dates
February 2nd - Applications available
April 3rd - Priority Scheduling Deadline
April 10: Last day to cancel with full refund (the $50 processing fee is nonrefundable)
Week of May 4th - First round of placements are sent out
May 8th - Payment due in full
May 15th - Last day to complete your application for scheduling consideration (incomplete applications cannot be considered)
June 8th - Summer sessions start
July 30th - Summer sessions end
